Job Overview

We are seeking two compassionate and dedicated Outreach Support Workers to join our team (second role advertised as Scarborough/Filey) .The ideal candidates will have experience working with people experiencing mental ill health and we strongly encourage applications from people with lived experience of mental ill health.

The Scarborough/Whitby role covers 2 projects that use the same service model but have different criteria on length of support. This role will be divided as follows:

18.5 hours per week working within Scarborough on the MindSide service. This role provides essential 1-2-1 support in the home and local community, alongside small group support to people aged 16 and over within our National Lottery Funded "MindSide" service. We delivery 16 sessions of 1-2-1 Recovery focused support alongside ongoing support groups (e.g. bereavement) and short term/ad hoc psychoeducation opportunities, within which you will have opportunities and support to pilot new interventions based on the community needs.

18.5 hours per week (1 day Whitby) will be providing the same model of support to people who have longer term mental health issues and therefore, there is no restriction on session numbers.

Duties

To provide community-based support to people with mental health problems so that they can become more independent, active and safe, and to support them to tackle the issues in their lives that may be obstructing good mental health and wellbeing. The core goals of our work are: People have improved mental health in the long term through addressing underlying causes, building resilience and coping strategies, expanding knowledge of self-care and healthy behaviours. People will build new connections and networks so they have the tools, people and places to help keep them well. People will have basic needs met that provide foundations for wellbeing. (Food, warmth, safety etc). This means advocating where needed for issues around housing, income etc. This includes:

* Undertake an assessment of service user needs
* Building a positive, trusting partnership with service users, supporting them in identifying the
* issues causing/contributing to mental ill health, barriers they face, goals and aspirations and the support needed to work towards the goals
* Support planning and goal setting in partnership with the service user
* Working with service users to increase self-sufficiency in terms of building resilience, improving mental health, improving overall health and wellbeing and developing a supportive network of organisations, individuals and places that form a long-term support system.
* Refer and support service users to engage in appropriate and meaningful activities, services and groups that will enable them to meet their goals
* Support service users to access other SWR Mind support such as befriending and employability
* Review support and achievements on an 8-session basis
* Facilitate and develop peer support workshops and support groups – supporting small groups of service users to come together, build skills and knowledge, share coping skills and develop a social network
* Train a small cohort of volunteers to support group and workshop delivery.
* Manage planned endings and move on from the service so that service users continue their Recovery journey with the appropriate support and structure in place
* Monitoring and evaluation – ensuring required information collected (using Lamplight CRM)
* Developing and maintaining appropriate partnerships/networks to ensure joint working with partners where beneficial to the service user
* Identifying new community resources and disseminating this information to the wider team
* Representing SWR Mind at planned events
* Be involved with wider organisation activities and events
* Undertake any other reasonable tasks as and when needed

Requirements

* Previous experience working in a support role is essential.
* Strong communication skills, both verbal and written, to effectively interact with clients and professionals.
* A compassionate approach with a genuine desire to make a positive impact on the lives of others.
* A full driving license and access to a car.

Pay: £24,875.00 per year
