MAIN RESPONSIBILITIES:

1. Management of Prescribing: Ensure that all requests for repeat prescriptions are actioned within two working days. Verify that all requested repeat items are due for issue. Add new medication following protocols, include correct dose instructions, and adjust quantities as needed. Liaise with local pharmacists regarding medication queries.
2. Investigate and resolve patient non-compliance issues, such as requests for early or irregular medication use. Address requests for non-repeated medication. Update patient records with drug data and ensure hospital discharge/outpatient information is accurately inputted.
3. Coordinate with Patient Advisors, District Nurses, Long Term Conditions team, and secondary care providers regarding follow-up interventions. Liaise with consultants, secretaries, Crisis Team, CPNs, Social Services, and other healthcare workers as required.
4. Monitor medication discontinuation and recommend appropriate changes. Rationalize medication use to achieve cost savings, including initiating generic alternatives.
5. Manage workload and staff allocation to meet service requirements. Work accurately and efficiently to meet deadlines. Support service improvement initiatives, audits, and monitoring to enhance patient safety. Work flexibly to cover absences.
6. Facilitate team integration, including training new staff members. Complete all tasks of Medicines Management Officers when required. Support the delivery of the ICB’s strategic medicines management plan.
7. Collaborate with Primary Care Pharmacists on projects related to Medicines Optimisation LES and QOF. Undertake practice-based project work under the guidance of the Scheme Lead to ensure cost savings. Assist the Medicines Optimisation Team with data collection, searches, and correspondence for prescribing changes. Identify patients suitable for electronic repeat dispensing and Medicines Use Reviews (MURs).
8. Promote better medication use through improved patient information. Attend relevant training courses and events. Use EMIS effectively to monitor specific medications.

Communication

The post holder will:

* Be empathetic to patients to ensure effective communication and prevent conflict.
* Share information within confidentiality and GDPR guidelines.
* Use verbal and non-verbal skills to resolve conflicts and manage anger.
* Handle telephone inquiries, providing answers or directing queries to a pharmacist or clinician.

Training and Development

The postholder will:

* Participate actively in learning and development activities, attending necessary courses and seminars.
* Identify personal development needs and discuss these with the manager.
* Provide support and guidance to team members, including training MMOs.
* Assist in training and assessment activities within the team, including evaluations.
* Participate in annual and mid-year performance reviews.
