Job Overview
PLEASE NOTE THAT WE WILL ONLY ACCEPT APPLICATIONS FROM STAFF CURRENTLY EMPLOYED BY ANEURIN BEVAN UNIVERSITY HEALTH BOARD

The post holder will have continuing operational and developmental responsibility for the management of the Community Resource Team (CRT) within the locality of Blaenau Gwent. They will facilitate on behalf of the ABHB, the Local Authority and other Partner Organisations the provision of services, care and support to people who fall within the frailty spectrum. To achieve this they will manage a complex range of specialist and multi professional services in an integrated, multiagency, multidisciplinary, cost effective, efficient and responsive manner.

Main duties of the job

The post holder will operationally lead developments within the Locality, clarifying and coordinating roles and responsibilities of staff, ensuring appropriate lines of communication and forums are in place to provide effective and professional decision making. They will also collaborate with Professional Heads for therapy, nursing and Social Care and other services across the health and local authority services, to ensure that strategic development plans meet the developing needs of the locality and achieve the Locality's Health and Social Care strategy. The post holder will manage the budget for the team and will assist the Locality to form and implement the future strategic direction for Frailty and other related services. They will also work closely with the Locality Leads in both Health and Social Care to ensure a strategic fit.

Qualifications & Registration
Essential criteria

* Relevant core professional qualification and registration such as Registered Nurse, Social Work, AHP
* Masters degree or equivalent experience
* Evidence of continuing professional and personal development

Desirable criteria

* Management qualification
* Evidence of leading change and projects with successful outcome
* Budget management experience and skills in achievement of financial targets

Knowledge

Essential criteria

* Up to date knowledge of current legislation, guidance and policy initiatives which impact on Health and Social Care services

Desirable criteria

* Knowledge and understanding of health and safety legislation
* Knowledge and understanding of performance monitoring systems, quality measures and professional governance arrangements

Experience
Essential criteria

* Evidence of managing, leading and evaluating community health or social care services or teams

Desirable criteria

* Evidence of managing and leading multidisciplinary, multi professional teams successfully bring together staff across different organisational boundaries including health and social care
