About the role:
1.Post is a 21 hour per week Support Worker
Working to a six-week rolling rota subject to change to meet needs of the service.
* There is a requirement to work shifts day and evenings also weekends and Bank Holidays.
You will be supporting Adults who have a Learning Disability to manage their own home as to the principal responsibilities detailed in the Job description.

What you will do:
* Supporting people with Learning disabilities to develop their skills and independence by demonstrating and modelling activities.
* Working alongside a dedicated staff team and other professionals in the service users' own home and other required venues.
* Undertaking practical tasks and supporting people with their personal care, domestic activities and socially in the community including driving service users to activities and appointments.
* Supporting service users to build and maintain family and friendship links and to reach goals and achieve personal outcomes.
* Participating constructively in regular staff supervision and appraisal sessions.
* Contribute to written assessments and Care Plans and attend Care Planning meetings compiling and keeping detailed records and reports.
* Participating/attending required training to develop, grow and gain skills to enhance the role.

Pay: £31,023.00-£32,024.00 per year
