We are looking for an experienced Band 6 Team Sister / Charge Nurse to join our Community Nursing team supporting patients across the Sheppey locality. This is an exciting opportunity to take on a senior clinical role within a supportive multidisciplinary team, delivering care to patients within their own homes and community settings.
You will work closely with the Team Lead and wider community teams to support the day-to-day coordination of services while delivering and overseeing high standards of patient-centred care.

Main Responsibilities:
As a Band 6 Team Sister / Charge Nurse, you will:
* Support the Team Lead with the day-to-day coordination of the community nursing service
* Provide clinical leadership and act as a role model to the wider nursing team
* Undertake holistic assessments and deliver evidence-based nursing care within patients’ homes and community settings
* Coordinate care plans and ensure continuity of care for patients with complex health needs
* Promote patient independence and support rehabilitation and recovery goals
* Work closely with the multidisciplinary team including GPs, therapists and social care colleagues
* Ensure a safe and effective service through appropriate clinical governance and risk management
* Maintain accurate and contemporaneous patient records
* Communicate effectively with patients, carers and the wider healthcare team
* Support junior staff, students and colleagues within the team
* Act in accordance with the NMC Code of Conduct
Ideal Candidate:
We are looking for a confident and organised nurse who is passionate about delivering high-quality care within the community.
You will:
* Hold a Nursing Degree
* Have current NMC registration
* Have experience working within community, rehabilitation or long-term condition services
* Demonstrate strong leadership and organisational skills
* Be able to work independently and as part of a multidisciplinary team
* Have excellent communication and interpersonal skills
* Be confident using IT systems including Microsoft Office (Outlook, Word, Excel)
* Be flexible to meet the needs of the service and participate in community service delivery
