You will be a member of a dynamic multi-disciplinary team providing Urgent Community Response and Discharge to Assess interventions in people's homes. Working with a well‑resourced, multi‑disciplinary team you will be supported to work in a blurred boundary way, and develop new clinical skills.
It is essential that you are a car driver and have a Full UK Driving License, reasonable adjustments will be considered.
