Overview
We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Assistant who is keen to deliver a range of quality care services to patients visited by the Community Response Hubs based at either Chester, Tarporley or Ellesmere Port. The team visits patients and their carers of all the Integrated Community Care Teams, across Western Cheshire.
Key Responsibilities
* Provide quality care services to patients in their homes.
* Carry out tasks such as simple dressings, catheter care, personal care and recording blood sugars.
* Provide care for palliative patients and work lone.
* Travel across the Western Cheshire footprint as required.
Key Requirements
* End‑of‑life care experience.
* Demonstrate ability to meet the travel requirement for the post.
* Access to transport for work purposes and must be demonstrated in the application.
* Work 4 × 9.5‑hour split shifts on a rota basis (08:30‑14:00 & 18:00‑22:00, including bank holidays and weekends).
Working Hours & Pay
37.5 hours per week. The Trust supports the Real Living Wage (RLW) hourly rate and will apply a pay supplement if the national A4C hourly rate falls below this.
