Main duties of the job
The Role
As an Integrated Care Team Nurse, you will work collaboratively as part of a nurse-led service to provide high-quality care to babies, children, and young people in their preferred location.

Our visionat Bluebell Wood is for every baby, child and young person with a life limitingcondition to access specialist palliative care, where and when they needit. You will join our fantastic teamwhere we all play our part in achieving our vision and creating an incredibleculture for everyone.
Our services
Ouruncompromisable support services are wide-ranging and bespoke to eachfamily. We provide symptom management,end-of-life care, post-death care, short breaks, counselling, sibling supportgroups, music therapy, home visits and more.
We supportfamilies across a large geographical area, including South Yorkshire, NorthNottinghamshire, North Derbyshire and parts of North Lincolnshire. The hospiceis easily accessible from Barnsley, Rotherham, Doncaster, Sheffield and NorthNotts. We have free secure on-site parking for families, staff and volunteersto use.
Every yearwe need to raise £6.7m to keep our doors open and only around 17% of our incomecomes from government sources, the rest must be raised through fundraisingactivity.
Job responsibilities
About the job
As a Bluebell Wood Integrated care team nurse, youwill
Workflexibly as a skilled member of the Care Services Team, providing high qualitynursing care to meet the diverse and individual needs of babies, children,young people and their families.
Deliverchild-focused, family centred palliative and end of lifecare in a variety of settings.
Lead shifts as the nurse incharge in the hospice and work autonomously in the community as required.
Support colleagues,students and trainees whilst on duty.
Clinical:
Assess, plan, evaluate and deliver high quality nursing care tomeet the diverse and individual needs of children, young people and theirfamilies.
Caring for children with a wide range of complex clinical needsand technology dependence (for example ventilation, tracheostomies and enteralfeeding devices).
Work with other members of the multidisciplinary care team toensure that plans of care are carried out to the agreed standard.
To ensure an ongoing welcoming, caring and safe environment isprovided for the children, young people and their families.
Identify and manage significant clinical changes in a child or youngpersons condition or well-being, using clinical reasoning to determinethe most appropriate course of escalation.
Establish therapeutic relationships with children, young peopleand their families.
Coordinate and deliver care after death at home and in thehospice.
Act as an advocate for the child, young person and family,providing them with information and support and directing them to the relevantsupport services where appropriate.
Communicate complex and sensitive information effectively to children,young people and their families, and other staff, overcoming any barriers tounderstanding and providing support during distressing or emotional events.
Develop and maintain clinical skills and knowledge necessary toprovide holistic, evidence-based nursing care. This includes completion of thehospice clinical skills competencies and expanded role responsibilities,following appropriate training.
Support the development of advance care plans.
Assist with ongoing education and training of colleagues, studentsand trainee practitioners.
To work closely with other teams across the hospice to support thecare of babies, children, young people and families.
Leadership:
Contribute towards quality and improvement initiatives.
Report near misses and untoward incidents, complaints, clinicalemergencies, injury or medicine administration errors, as outlined in thehospice policies and procedures.
Lead shifts, ensuring team working principles todeliver a high standard of holistic care, providing support and guidance tostaff on shift.
Provide regular clinical supervisionfor designated staff and raise any concerns or needs to those who have linemanagement responsibility.
Education:
Actively identify training needs, assistingin the education, training and clinical supervision of colleagues, students andtrainees.
Supervise and orientate learners and newstaff, supporting the induction process.
Participate in surveys, audit, researchand facilitating clinical trials as required.
Assess students on placement, in linewith the requirements of the Nursing and Midwifery Council.
Take responsibility for personal trainingneeds, ensuring required competencies are met and signed off in a timely way.
Governance:

Ensure a consistent approach to all aspects of care delivery byfollowing protocols and prescribed care, recommending changes to advancepractice.

To deputise for Team Leads in governance groupsas needed.
Professional requirements:
Comply with the requirements for NMC Revalidation, identifying ownlearning needs and ensuring own professional development is maintained bykeeping up to date with practice developments.
Participate in clinical supervision and appraisals.
Successfully complete required competences as defined by BluebellWood.
To comply with the NMC Code of Conduct at all times.
