We have an exciting opportunity to join the Buckinghamshire Team. You must be able to drive and have your own transport.

We have 1 x 30 hours Monday - Friday working 9am - 3:30pm

Salary - £19,760 - £22,960 for part time 30 hours

The Health and Wellbeing Coach provides personalised support to individuals to empower them take control of their own health and wellbeing with a focus on lifestyle behaviour change, inclusive but not limited to the core areas of:

1. Healthy Eating and Nutrition
2. Smoking Cessation
3. Physical Activity
4. Weight Management
5. Alcohol consumption
6. NHS Health Checks (outreach)


As a Health and Wellbeing Coach, you will facilitate people in establishing and attaining health-promoting goals, set by the person to change lifestyle-related behaviours, with the intent of reducing health risks, improving self-management of chronic conditions, and improving quality of life. The Health and Wellbeing Coach is also responsible for delivering face-to-face, telephonic, and digital health coaching support to individuals from a wide range of community and healthcare settings via a combination of one-to-one and group-based support in line with need, location and offer.

To provide a health coaching service for eligible participants that supports people to gain and use the knowledge, skills, and confidence to become active participants in their health and wellbeing so that they can reach their self-identified health and wellbeing goals, including, but not limited to:

7. Triage and assessment
8. Agenda setting
9. Lifestyle behaviour change support to (e.g.) stop smoking, move more, achieve a healthy weight/diet, reduce alcohol consumption levels.
10. Problem solving
11. Motivational interviewing
12. Goal setting
13. Signposting


Delivery multi-channel, personalised health coaching support to individuals and groups in line with their needs and preferences including:

14. Face-to-face, telephonic, and digital health coaching
15. Share helpful and supportive resources in line with goal, stage of journey, confidence, and motivation.
16. Advocate the use of behaviour tracking tools (online or offline)
17. Appraise individuals and population caseloads daily to ensure participants are provided with the right intensity and frequency of support to drive sustained behaviour change.
18. A strength-based approach, that maximising opportunities to utilise local community assets and efficient utilisation of the wider health and wellbeing economy.


To deliver services in line with quality standards, national / government guidelines, and role boundaries, with the ability to identify and act accordingly when coaching is not appropriate and being able to sensitively discuss alternatives or escalate appropriately – inclusive on supporting onward referral to wider services.


Support with outreach community engagement work inclusive, but not limited to:

19. Screening services (e.g., NHS Health Checks)
20. Community and stakeholder events
21. External meetings and networks
22. Roadshows and pop-ups

To deliver services in line with quality standards, national / government guidelines and role boundaries, with the ability to identify and act accordingly when coaching is not appropriate and being able to sensitively discuss alternatives or escalate appropriately – inclusive on supporting onward referral to wider services.

Work in line with business and contract performance requirements including:

23. Implementing Standard Operating Procedures (SOP)
24. Fulfilling policies and quality management framework expectations including (but not limited to), data protection, information security, Health and safety and safeguarding.
25. Participate in regular audits and performance observations, with a pro-active commitment to providing high quality services and continuous professional development.
26. A proactive approach to supervision and training with a demonstrated commitment to personal development, achievement of quality standards, high performance in line with agreed objectives and Key Performance Indicators KPIs

To ensure accurate reporting and data collection in line with contractual and case management system requirements to ensure:

27. Caseloads can be managed efficiently, effectively and at scale.
28. Team members are able to support in population behaviour change as required and in line with need
29. Wider informal and formal care givers / specialists can complement and enhance the behaviour change journey
30. Service users / participants only have to tell their story once and where behaviour changes occur sequentially or concurrently, we are able to offer a comprehensive and coordinated supportive services
31. Data audits accurately capture activity, productivity, utilisation, and outcomes.

Qualifications & Experience


Essential

32. Relevant health coaching qualification or an accredited health coaching skills programme.
33. A minimum of six months of direct health coaching delivery
34. Experience in delivering behaviour change interventions for specific lifestyle related issues (e.g., smoking cessation, physical activity, NHS Health Checks, weight management, alcohol reduction)
35. Evidence of supporting vulnerable individuals through a change process
36. Experience of caseload management demonstrated via the use of a Case Management System
37. Experience of inputting and processing data where accuracy, targets and time deadlines are operating in the workplace.
38. Experience of supporting vulnerable individuals through a change process
39. Evidence of continuous professional development to ensure the delivery of the most recent and up to date health coaching provision to the correct quality standard.
40. Experience of working in a health, community or social care environment or good understanding of the issues around maintaining a healthy lifestyle Desirable:
41. Membership of professional body (ICF, EMCC, AoC, UKHCA)
42. Experience of managing and supervising individuals and teams.
43. Educated to degree level in a relevant subject e.g., Psychology, Behavioural Science etc.
44. Experience of supporting people remotely / telephonically / digitally
45. Evidence of working with individuals with long term health conditions

Individual Competencies

Essential

46. A strong understanding of the social determinants of health
47. A strong understanding of population-based approaches including segmentation and risk stratification.
48. A strong understanding of behaviour change principles and methodology including the COM-B model and behaviour change wheel.
49. Excellent oral and written communication skills with people from a wide variety of backgrounds.
50. Demonstrable core skills and competencies as set out in best practice standards including:
51. Select and apply a range of health coaching models, conversation frames and techniques.
52. Detailed knowledge of the core concepts and principles of personalisation, patient activation, health behaviour change, self efficacy, intrinsic motivation, and assets-based approaches.
53. Detailed understanding of self management support and associated techniques.
54. Advanced skills development incorporating practice, reflection and planning for the application of learning to practice.
55. Excellent internal and external stakeholder engagement and management.
56. Good understanding of principles of confidentiality and safeguarding.
57. Strong written and verbal communication skills with the ability capture essential information that supports effective case management.
58. A strong track record of interpersonal relationships including the ability to build, develop and maintain relationships.
59. Effective caseload management inclusive of accurate data collection, data entry, timely recording, and delivery against agreed Service Level Agreements (SLAs) and Standard Operating Procedures (SOPs).
60. Confident in the use of evolving digital technologies to support people through behaviour change processes.
61. The ability to manage time independently and effectively and work to deadlines.
62. Effective safe and sensitive data management in line with information security standards.
63. Excellent Microsoft Office skills and ability to learn quickly when using news systems and processes.
64. Commitment to personal development / training.
65. Commitment to flexible working (including evenings and weekends), ensuring the service is available to all residents including, those who work during the day, shift workers and those with children of school age.
66. Commitment to supporting overall team/contract performance to the highest standards, undertaking a range of duties in line with business requirements.
