The community team of West Essex is part of the wider Hertfordshire and West Essex Integrated Care System, supporting out of hospital care for West Essex residents. To provide leadership to a multidisciplinary Integrated community care team to deliver patient centred care to people with a focus on rehabilitation. This will involve helping patients to engage as independently as possible in activities which enhance their health & wellbeing. Interventions will be carried out within patients own homes, 24 hour care facilities and clinics. To be responsible for ensuring the delivery of high-quality person-centred services. day to day management and leadership of therapy and assistant practitioners. To provide senior clinical advice, leadership and support ensuring the needs of the service are met by exercising and demonstrating high levels of clinical judgement, critical risk assessment and advanced decision making skills.Flexible working is required, as at times you will be expected to work across sites in our other West Essex localities depending on service needs. Main duties of the job • To act as a senior decision maker to guide and support staff with complex patient cases. • To assess, plan, implement and evaluate specialist treatment and care and/or rehabilitation programs to people, promoting independence and autonomy; working within a multidisciplinary team. • Provide expert clinical holistic assessment and evidence based pro-active and reactive health care interventions to support patients and their carers to achieve maximum health welling and independence • To participate in duty clinician cover, triage and MDT/allocation meetings where clinically appropriate. • To act at all times as an advocate for service users, carers and relatives. • To be responsible for standards of care within the service area; to evaluate clinical care and develop actions with relevant line managers where required. • To have overall responsibility for caseload management through continual monitoring and data collection. • To maintain a professional portfolio and demonstrate that practice is up to date and evidence based through regular management and clinical supervision/mentorship. • Day to day management and leadership of therapy and at time nursing practitioners and other staff in the multi-disciplinary team. Job responsibilities Communication Skills: • Using verbal, non-verbal and written communication tools ensure that patients and carers have an understanding of their condition and the role and aims of Therapy in relation to this. • To be confident in their own diagnosis and be able to communicate that effectively to others (including medical doctors, other Allied Health Professionals etc). • To share information in a timely manner with more senior staff if there are any concerns about a patients safety, condition or prognosis. • To urgently report and take, appropriate action if there are any concerns about staff and/or patient safety • To represent the community service in meetings with other professionals with an aim of accurately contributing and reporting back relevant information to their line manager. • Maintain confidentiality at all times, consistent with Trust policy. • Maintain accurate clinical records in accordance with professional and trust standards. • Use IT skills accurately in patient notes and report writing. Professional Skills: • The staff member must be able to adapt their communication, treatment, information and advice to meet the needs of individual patients. Patients seen will range from the vulnerable to those who are extremely well informed about their condition. • The staff member must be sensitive to patients own cultures, beliefs and emotions, especially when asking them to undress for treatment n during assessments or treatments. • To be professionally and legally accountable for all aspects of own work including the management of patients in your care. • To take a lead for specific projects within your area of work to enhance patient care and service delivery. • To train, supervise and performance manage junior staff, technical instructors, assistants and students. • To effectively manage and prioritise your own workload within the time available including the prioritisation of patients to be treated along with other commitments in the day. • Actively participate in Continuous Professional Development and maintain a portfolio recording learning outcomes. • Undertake the measurement and evaluation of your work and current practices and participate in research and development through the use of Evidence Based Practice projects, audit and outcome measures. To make recommendations for change as a result of this process. • Demonstrate a sound understanding of Clinical Governance and Risk Management and apply this understanding to work situations. Education and Research: • Maintain appropriate and up to date knowledge and skills by undertaking continuing education in accordance with personal and service needs. • Undertake appropriate education and training for professional development, to maintain own knowledge and credibility. • Identify training needs and provide training and mentoring support/preceptorship to designated team members, pre and post registration students health care professionals and others. • To provide training and support to clinical colleagues. • Ensuring that learning opportunities are effectively utilised. • To actively participate in clinical supervision on a regular basis in line with Trust guidelines. • To participate in annual appraisal and maintain a personal development plan. • To participate in research and audit to ensure the development of effective and innovative practice and maintenance of standards. In addition to the above duties you will also be expected to perform the below key activities in line with your job role; - Complete mandatory training in line with Trust policy and procedures - To participate in the staff appraisal process and to undertake for any staff you manage - To keep yourself updated on all matters relating to Trust policy - To provide management supervision where appropriate - You will be expected to work collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7-day a week, 24-hours a day service for 365 days a year working shifts, where appropriate and operationally required. Education/Qualifications Essential • Recognised qualification /degree in Physiotherapy • Health Care Profession Council Registration as a Physiotherapist Desirable • Member of the CSP Experience Essential • Significant experience of working within a community setting • Experience of leadership • Experience implementing change in clinical practice
