Primary Care Nurse Practitioner/Advanced Nurse Practitioner

Main duties of the job
As a Registered Nurse Practitioner you will be expected to be able to perform core clinical nurse practitioner responsibilities alongside delivering excellent women's health services.
What We Are Looking For
* Qualified and registered Prescribing Nurse Practitioner
* Proven interest and competencies in women's health, including contraception, menstrual disorders, menopause care and sexual health advice
* Positive and committed
* Delivering a high standard of patient care, using advanced, autonomous clinical skills and a broad in-depth theoretical knowledge-base.
* Assessing, diagnosing and treating patients with both minor and complex needs, initiating referrals as appropriate and planning clinical care accordingly.
* Performing safe triage of patients when necessary via telephone or face to face.
* Prescribing medication (if qualified) appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
* Providing information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
* Prioritising health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
* Provide leadership, education and training to other healthcare professionals and staff.

Oakfields Health Group is a 2-Partner dispensing practice with 2 sites at Hamsterley Colliery and Dipton, providing excellent quality healthcare to 7700 patients. We are semi-rural, located in a beautiful part of the region where Gateshead meets Northumberland and County Durham. We have excellent transport links from the A1.
Our clinical team includes 4 salaried GPs and a nursing team of nurse practitioners, practice nurses and healthcare assistants. The clinical team is supported by experienced administrative and managerial teams and we work closely with our PCN colleagues. We benefit from having a pharmacist, pharmacy technician, GP assistant as well as having access to First Contact Services.

Job responsibilities
Key responsibilities
Provide same-day and routine clinics for undifferentiated presentations within scope and protocols, with GP support where required.
Deliver women's health services: contraception (including LARC if trained), menstrual disorders, gynae examijnations, menopause care, cervical screening, sexual and reproductive health advice, and postnatal support.
Deliver contraception and sexual health services in line with FSRH standards and local commissioning arrangements; contribute to PCN womens health hub pathways and timely triage/referral where appropriate.
Undertake long-term condition reviews icluding those where aligned to womens health (e.g. CVD risk in menopause, gestational diabetes follow-up).
Participate in QI, audits, and service development to improve access, outcomes and reduce health inequalities.
Provide clinical leadership to nursing/HCA teams within scope; support training, supervision and protocol development.
Accurate, contemporaneous record-keeping and contribution to safeguarding, IPC, and medicines management governance.
Work to local/national guidelines (including FSRH guidance) and support CQC always-ready standards across Safe, Effective, Caring, Responsive and Well-led domains.
Person Specification
Qualifications
* Current registration with the NMC
* To be considered for an Advanced role, the relevant Masters or accreditation although applications from a (non-Advanced) nurse Practitioner would also be accepted
Experience
* Experience of working within a nursing team.
* Experience of delivering women's health services
* Experience of dealing with a variety of patients.
* The ability to work with a high degree of autonomy and have sound clinical reasoning and decision making.
* Experience of working in primary care
* Experience of SystmOne.
