Asthma Clinical Nurse Specialist

An exciting opportunity has arisen with the Asthma Clinical Nurse Specialist Team.
Duties will be to manage own defined caseload of patients within the agreed scope of practice and taking responsibility for continuity of care acting as an independent practitioner. This will include assessing patients often with highly complex or chronic conditions, providing specialist advice across professional boundaries and planning and implementation of care, running nurse led clinics and comprehensive follow up services; and ensuring a smooth transition for patients through the relevant pathway.
Main duties of the job
* Manage a caseload of patients undertaking physical assessment of the patient and will assume responsibility for requesting surveillance investigations relevant to the specialism (list areas of competence required), evaluating results and instigating any necessary going action.
* The post holder will maintain seamless lines of communication with the patient, as well as between secondary and primary care. The post holder will oversee management of resources ensure appropriate use of resources to promote effective patient care and the achievement of national standards.
All clinic work will be delivered under the indirect supervision of a named consultant.

Person Specification
Education and qualifications
* Current NMC Registration
* Attained or working towards 1st Degree in health related subject
* To have completed the national PEAP programme for practice supervisors or equivalent teaching and assessing qualification for those with ENB998.
* Non-medical prescribing V300 course.
* Education for Health or ARTP/BTS Certificate in Spirometry or equivalent
* APACS
* Evidence of relevant study
Experience
* Demonstrable post registration experience
* Demonstrable experience in relevant area of practice (respiratory medicine) or equivalent area
* Effective leadership skills
* Evidence of applying research and audit in practice
* Experience of successful multiagency working
* Experience within Palliative Care
* Experience with managing Home Oxygen
Skills and knowledgs
* Able to work effectively as part of a multidisciplinary team, crossing professional and organisational boundaries
* Ability to work autonomously
* Well-developed effective interpersonal and communication skills
* Ability to prioritise workload
* Computer literacy skills including knowledge of databases, power point and word processing and patient record systems
* The maintenance of up to date knowledge of local and national developments in nursing, medical practice and policies.
* Experience developing integrated care pathways, protocols and guidelines
* Evidence of previous health promotion work
* Demonstrates advanced clinical practice
