Discharge Facilitator
Band 6, Fixed term - 12 Months
The post holder will be responsible for case managing and facilitating safe and effective discharges of patients from hospital to a community setting through a multidisciplinary approach. They will coordinate and oversee Discharge Navigators within their sub-team, complete assessments of patients' needs and support the discharge plan. The post holder will be proactive in identifying concerns or issues that may delay effective discharges and work with the Transfer Of Care Hub Team (TOCH) to overcome and limit these. The successful candidate will work closely with all ward teams ensuring appropriate referrals are seen and made to local authorities and preventing unnecessary, avoidable delays for patient discharge. This role will extend to support and ensure that patients who have been discharged feel supported and have received the support that was requested. You will need to have critical thinking skills and be able to communicate effectively. This role will be full time across 7 days so flexibility will be required.

Responsibilities
* Be competent within their scope of practice and provide compassionate care and therapy to agreed quality standards.
* Maintain momentum of discharge planning throughout the entire process; support and work in partnership with other members of the Complex Discharge Team, Ward MDT and Community partners, constructively challenging where appropriate decisions with regards to discharge planning.
* Undertake baseline assessments, including Mental Capacity Assessments and Care Act assessments, in partnership with other members of the MDT and ensure that the patient is kept informed of their discharge plans at all times.
* Represent the Transfer Of Care Hub Team at ward daily board rounds.
* Provide effective and timely communication of discharge actions using existing communication tools.
* Undertake a range of duties without direct supervision and report back on delegated duties to the ward team and case manager.
* Work across multi-professional disciplines and make referrals for additional assessments and treatments as required, supported by the case manager.
* Promote the use of the discharge lounge.
* Have supervisory responsibility for the Discharge Navigators.
Qualifications and experience
Education and qualifications
* Education to Clinical Qualification or Social Work Qualification
Knowledge and understanding
* Knowledge of patient administration systems (Cerner).
* Able to communicate effectively with a range of stakeholders, patients, relatives and carers.
* Understanding of how complex organisations work.
* Able to work effectively as part of a team but also able to work under own initiative.
* Able to take instruction from senior staff.
Desirable
* Literate in IT systems for patient information and communication.
Experience
* Previous experience in a clinical role in discharge planning or equivalent.
* Experience in an acute hospital environment.
* IT literate.
Desirable
* Worked in the discharge setting.

* Ability to manage own caseload.
* Good time management and organisational skills.
* Ability to evaluate own performance and recognise limitations.
* Flexible and open minded; enthusiastic with professional appearance.
