Overview
A Band 5 Community Nurse position is available within the Haltemprice Community Nursing and Conditions Team. This permanent role is for 37.5 hours per week, scheduled over 7 days. The position allows for flexible working hours. The selected candidate must possess strong communication skills and be able to adapt to the changing demands of community nursing. IT literacy is also required. The role requires a car and the ability to travel between various work locations, as well as for attending training and meetings.
Responsibilities
* Deliver prescribed care as indicated in the patient care plan and promote best practice of the District Nursing Service.
* Undertake within the scope of practice initial patient care assessments and plan care.
* Provide comprehensive community nursing care to clients requiring long-term conditions management, complex wound care, or palliative care.
* Deliver services in patients’ homes (including residential care) and designated treatment rooms.
* Work within a multidisciplinary team and collaborate with local providers and other healthcare organisations in Hull and the East Riding.
Requirements / Qualifications
* Strong communication skills and the ability to adapt to changing demands of community nursing.
* IT literacy and the ability to use relevant systems.
* Own car and ability to travel between work locations and to attend training and meetings.
