<div><p>CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!</p><p>We are a dynamic and forward-thinking team looking for an experienced and motivated Band 6 Registered Nurse to join our established Watford and Three Rivers Community Nursing Team as a Deputy Team Lead.</p><p>This role offers an excellent opportunity to co-lead one of our neighbourhood teams, working closely with and supported by experienced Team Leaders.</p><p>The position is varied and rewarding, with responsibility for managing a patient caseload. You will assess, plan, implement and evaluate evidence-based care for patients in their own homes, helping to support independence and improve outcomes.</p><p>We are particularly interested in candidates with previous community nursing experience.</p><p>We are looking for someone who is committed to collaborative working, approachable, and an effective communicator. You will be confident working both independently and as part of a team, and open to new ideas and ways of working.</p><p>As this is a community-based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).</p><h3>Main duties of the job</h3><p>This is an exciting time to join the Community Nursing service in West Hertfordshire. We are developing new community pathways which will support the delivery of more care in peoples homes, and to enable us to do this we have a range of new learning and development opportunities available through the CLCH Academy.</p><p>Key duties within the role include:</p><ul><li>To set and maintain standards of care and deploy, co‑ordinate and supervise the nursing team and learners as required.</li><li>To provide leadership and management to a team of community nurses and other staff covering the four localities across Herts Valleys.</li><li>To be responsible for the teams assessment, planning, implementation and evaluation of individualised nursing care to patients living in the community. The service is provided seven days per week 8am-6pm.</li><li>To be responsible for the teams maintenance of patient and other records.</li><li>To participate in the long‑term management of patients within the locality.</li><li>To have continued responsibility for a defined caseload.</li><li>To be responsible for the development and maintenance of competencies within the team.</li></ul><h3>Person Specification</h3><h3>Education and Qualifications</h3><ul><li>Registered Nurse with current UK NMC registration.</li><li>Practice Supervisor/Assessor or equivalent.</li><li>Specialist Nursing Practice (District Nursing) Degree or will undertake training.</li><li>Nurse Prescribing or will undertake training.</li><li>Evidence of personal development.</li><li>Knowledge of NMC code of professional conduct.</li><li>Independent Prescriber experience.</li><li>Relevant courses in Long Term Conditions Management.</li><li>Course of Top to Toe examination/diagnosis experience.</li></ul><h3>Experience</h3><ul><li>Experience as a Community Nurse.</li><li>Management of a team.</li><li>Management of change within a complex environment.</li><li>Experience of teaching others.</li><li>Proven clinical skill at Senior Community Nursing level.</li><li>Experience of participating in investigation/management of complaints.</li><li>Knowledge of recent developments in NHS.</li><li>Knowledge of evidence based practice.</li><li>Knowledge of NMC code of professional conduct.</li><li>Evidence of participating in clinical audits in the workplace.</li></ul><h3>Other Requirements</h3><ul><li>Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).</li></ul><h3>Disclosure and Barring Service Check</h3><p>This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.</p><p>£41,957 to £50,387 a year per annum, inclusive of HCAS</p></div>
