The post-holder will work within the Rheumatology and T/O Service. This service provides high quality care to patients from North Staffordshire and beyond following a range of rheumatoid conditions including trauma injury, deterioration in long term conditions and other complex presentations seen in this patient cohort.

This clinical remit of this role is supporting therapy services for the Rheumatology/T&O service based at the Haywood Hospital. This is a dynamic post that requires the post holder to work in an integrated approach working collaboratively with the patients; their carers and the wider multi-professional team to determine effective treatment plans and patient focused goals. The core value of the role is to improve the health and well-being of the population served.


Main duties of the job

* Work with registered staff to provide a service for a defined population of service users in a variety of settings across the Partnership Trust including the community and hospitals.
* Be skilled in undertaking delegated tasks in the implementation of care/support packages and provide feedback working under the direction of a registered practitioner.
* Support the registered practitioner in the preparation and delivery of care/support packages including any administrative duties.

Detailed Job Description And Main Responsibilities

To be aware of the Health and Care Professions Council’s (HCPC) and CSP guidance/codes of conduct.

To establish and maintain effective working relationships both within the trust and with external organisations in order to meet service requirements and support and demonstrate the Trust’s equality values.

To Contribute To The Assessment And Implementation Of Programmes Of Treatment Care/support For Individuals, Families And Carers Under The Guidance Of a Registered Practitioner. The Care/support Plans May Include The Following

* Promoting independence/self management and maintaining safety; Increased functional ability
* Improvement in health and wellbeing including mental health; Interventions in disease/condition management
* Prevention and reducing of health inequalities
* Admission avoidance
* Early supported discharge

Contribute to case management/ treatment care plans.

To maintain and develop skills and knowledge to deliver a range of Interventions under the guidance of a registered practitioner and set an example of good practice to other team members including skill sharing. To provide physiotherapy support to service users within specified areas.

To apply skills, knowledge and competences to recognise any significant change in the holistic needs of the service user/carer and feedback to the registered practitioner.

To maintain contemporaneous and accurate records as required by the trust and in line with HCPC and CSP standards.

To make resources for use in therapy/care taking direction from registered practitioners as required.

To participate in regular supervision.

To contribute to the process of induction for new team members and demonstrates duties to others as required.

To identify own training/educational needs as part of the supervision and appraisal process in conjunction with registered practitioners. Attains and maintains an agreed level of competence through ongoing training and development.

To assist in general administrative duties as required including dealing with messages, photocopying, filing, receptions duties and appointment management.


Person specification

Qualifications

Essential criteria

* GCSE (A-C) English and Maths or equivalent
* NVQ Level 2 in related subject area or appropriate level of experience and knowledge.
* Evidence of/willing to undertake training relevant to the post.
* Evidence of continuing professional development

Desirable criteria

* Qualifications and Credit Framework QCF/NVQ/apprenticeship level 3 in related subject or willingness to work towards
* Care Certificate completed

Experience

Essential criteria

* Relevant placements/experience to the role.
* Experience of working as part of a team

Desirable criteria

* Specific experience including supporting or contributing to assessment and management of individuals with compromised functional ability.
* Supporting service users, families and carers in dealing with the impact of compromised functional ability
* Experience of working in Physiotherapy.
* Working in a multidisciplinary team.

Skills

Essential criteria

* The ability to demonstrate a basic knowledge of physiotherapy and its purpose.
* Understanding of Information Governance
* Data Protection, HCPC/CSP
* Safeguarding Vulnerable Adults/Children.
* Ability to manage time effectively

Desirable criteria

* A basic understanding of medical conditions
* Use of clinical healthcare records E.g. Rio/ Careflow
