Job Summary
Under general supervision, provide patient navigation services related to care coordination, referral management, service continuity, and accessing resources. Assist patients in navigating the healthcare system, re-engaging in care, and understanding health education. Facilitate patient compliance with ambulatory care, HIV care, discharges, follow-up, home care, and community resources.
Essential Functions
* Manage care transitions from ED, inpatient, SNF, specialty to primary care (20%)
* Address barriers to facilitate care coordination, improve patient experience, and ensure retention-in-care (10%)
* Track and document referrals to promote team awareness and patient safety (10%)
* Gather clinical background and referral needs, providing appropriate information (10%)
* Coordinate with organizations and insurance for prior approvals, sharing medical info as needed (10%)
* Assist patients with problem-solving related to healthcare, financial, or social barriers, acting as an extender for the care team (5%)
* Serve as a point of contact for patients and families, advocating for timely approval of necessary services (5%)
* Identify and utilize community resources; serve as liaison for care providers and families (10%)
* Collaborate with care team to implement patient care solutions, reminding patients of appointments (10%)
* Track referral processes, ensuring follow-up and barrier resolution (5%)
* Use motivational interviewing and teach-back methods to enhance patient understanding of care plans (5%)
Education
* High School Diploma or GED required
Work Experience
* 1-3 years in a clinical care setting required
Knowledge, Skills, and Abilities
* Intermediate knowledge of correctional/justice systems and community resources in Denver
* Ability to support community engagement and connect individuals involved with the criminal justice system to Denver Health
* Strong communication skills; empathetic approach
* Experience working with disadvantaged populations, including those experiencing homelessness and recent immigrants
* Bilingual in English/Spanish preferred
* Knowledge of Epic and Microsoft Office
