Akar Care provides high-quality care for the elderly. As a Care Home Assistant Practitioner (CHAP), you will support the physical, mental and emotional needs of our residents and work alongside fully trained nurses to deliver individualized care. You will play an integral part of our united and extended care team.
Responsibilities
* Assist nurses with day‑to‑day nursing duties so they can focus on complex care needs.
* Build and maintain strong relationships with residents, their families and the wider care community.
* Support residents in achieving the highest quality of personalized medical and clinical care.
* Cooperate with colleagues and external professionals to preserve the identity and dignity of residents.
* Follow the care plan and ensure residents receive a dignified, person‑centred approach.
