* Responsibilities:
o Completing Care Act Assessments to facilitate smooth transitions for service users leaving the hospital.
o Supporting discharges from intermediate care bed/inpatient rehab settings.
o Collaborating with health colleagues for discharge planning and ensuring the right pathways for individuals.
o Promoting a strengths-based approach.
o Working closely with individuals, families, carers, and community services (reablement, telecare, care homes, domiciliary home care providers) to promote independence and safety post-hospitalization.
* Requirements:
o Relevant Level 3 Qualification in Health and Social Care.
o Experience in producing quality assessments.
o Collaboration with various agencies.
o Understanding of the current Discharge to Assess process.
o Collaborative work with health colleagues in acute and intermediate care settings.
