About the role
For this role you will predominantly work with CKD inpatients and outpatients, and spend time with dialysis patients. The post offers a variety of clinical work and the opportunity to work within an established MDT.
The successful candidate will be based at Northampton General Hospital and will support the renal team as part of collaborative working across the University Hospitals of Northamptonshire group.
Main duties of the job
* Telephone clinics for advanced kidney care, some clinics may be attended in person and off main site.
* Providing care for dialysis patients and advanced kidney care patients who may require specialist nutrition advice.
* Undertaking clinical assessment of patients with complex presentations, multi-pathologies within acute inpatient and outpatient settings and providing effective monitoring.
* Formulating individual management and treatment plans for patients using advanced clinical reasoning and developing comprehensive discharge plans including advice on appropriate ACBS prescribable enteral feeds and supplements. This includes secondary prevention.
* Working collaboratively with the wider multidisciplinary health care team to ensure delivery of coordinated care and to provide specialist advice and attend case conferences, network meetings as appropriate.
* Evaluating patients' progress and measuring outcomes, and contributing to and implementing service improvement work.
* Using a range of verbal and non-verbal communication tools to communicate effectively with patients to implement treatment plans. This will include patients who have difficulty with understanding communication.
* Providing advice and training to patients, carers, colleagues and other healthcare professionals to promote an understanding of the aims of dietetic treatment to ensure a consistent approach to patient care.
* Managing clinical risk within own caseload.

Clinical skills and knowledge
* Has sound dietetic knowledge, skills and experience – ideally some renal knowledge.
* Displays competent and wide range of communication skills and able to use behaviour change approaches.
Professional skills
* Demonstrates organisational and problem solving skills, team player.
