Overview

Are you a passionate and caring nurse with at least two years' experience in the community, looking to progress and develop your clinical and leadership skills in a dynamic community nursing team? A vacancy has arisen within the Unplanned Care Team, which has an innovative approach to nursing care. It involves quick responses to patients in their home to assess, plan, manage and evaluate the presenting issue, prevent unnecessary hospital admissions, and help to facilitate early hospital discharges. You will support the clinical band-7 with the daily leadership and management of the team and liaise with the wider multi-disciplinary team. We are looking for a highly motivated applicant who is interested in meeting the challenge of working in the community with patients in their home setting. The Unplanned Care Team works with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients. You will work on a rotational basis as part of a seven-day service covering 24 hours a day with agile technology to help you deliver timely, high-quality care.


Responsibilities

* Assess, plan, implement, monitor, and review patient care in the community and in patients\' homes.
* Support the district/clinical band-7 with daily leadership and management of the Unplanned Care Team.
* Liaise with the wider multi-disciplinary team to achieve the best patient outcomes.
* Work as part of the community nursing team in unplanned care, ensuring high-quality health care is delivered to patients in their own home.
* Deliver care across a seven-day rota (24-hour shift pattern) and work agilely across the service.


Qualifications and Experience

* Senior staff nurses will have a minimum of 12 months' community nursing experience; other applicants should have at least two years' community nursing experience.
* Excellent clinical skills and leadership qualities; ability to implement high standards of patient care and ensure a positive patient experience.
* Ability to work with a variety of disciplines within the multidisciplinary team to achieve best outcomes for patients.
