Main duties of the job
The postholder will collaborate with multidisciplinary teams to support patients and families, ensuring services address the whole person and their social needs, and act as a link between health and social care.
You will provide emotional and psychological support on the inpatient unit and in day services, utilize social work skills for complex discharges, and support assessments related to MCA and DOLS.

Job responsibilities
Main duties
* Manage a complex caseload, work autonomously, and collaborate effectively within the multidisciplinary team.
* Maintain accurate records and support social work practice with supervision.
* Provide advice, education, and support regarding the needs of patients with advanced illnesses and their families.
* Ensure compliance with policies, confidentiality, and cultural sensitivity.
* Contribute to psychological support development and support staff involved in MCA and DoLS assessments.
Clinical
* Assist with conflict mediation, utilize talking therapies, act as an advocate, and provide advice on financial and legal matters.
* Conduct holistic assessments, support safety and risk management, and facilitate external agency links.
Education
* Participate in training, support students, and educate local authority teams about palliative care.
* Lead safeguarding training and clinical supervision groups.
Personal Development
* Work within professional standards, stay updated with best practices, and network with other teams.
* Assess own development needs proactively.
General Responsibilities
* Represent the hospice at meetings and events, and stay informed on relevant issues.
People and Departmental Management
* Support administrative volunteers within the team.
