Role Overview
Under the direction of the Clinical Coordinator and/or the Clinical Director, the Permanency Coordinator is responsible for developing and coordinating permanency plans to meet the social, emotional, and developmental needs of youth who have experienced complex early childhood trauma and/or who are living with mental health diagnoses.
How You'll Be Making a Difference
1. Participate in Massachusetts Permanency Practice Training Series.
2. Work directly with youth to identify and engage their family/identified supports in the permanency planning process.
3. Search for and engage a youth’s family network and others in the youth and family’s natural support system, considering steps needed to build a team around the youth and permanency readiness.
4. Utilize online search strategies and case record mining to identify potential members of a youth’s permanency team.
5. Organize and lead Youth Guided Family Driven Team meetings for individual youth.
6. Communicate frequently with treatment team members, including DCF, regarding progress.
7. Provide case management services, act as a liaison with collaterals, and establish relationships supporting youth and family growth.
8. Advocate for families, teach parents how to advocate for their youth, and help develop their navigation skills within social services and behavioral health systems.
9. Assist with initial assessments and evaluation of new youth and families, and help design permanency-focused treatment plans.
10. Use best practice tools with youth and families to support permanency outcomes.
11. Collaborate with clinical and program staff to assess youth permanency needs and develop supportive programming.
12. Participate in permanency consults across programs and collaborate with the Center for Permanency.
13. Facilitate permanency trainings and provide feedback on intervention implementation.
14. Supervise residential counselors and staff involved in permanency initiatives.
15. Maintain active communication with treatment teams and document all activities timely and accurately.
16. Be flexible with scheduling, including some evenings and weekends, and ensure availability of a valid driver’s license and personal vehicle.
Qualifications
1. B.A./B.S. in human services, psychology, or social work preferred.
2. Experience working with youth in residential or community settings.
3. Supervisory experience is helpful.
4. Proficiency in Microsoft Office and ability to learn new software.
