Job Overview
We have exciting opportunities for newly/recently qualified physiotherapists in our established and varied Intermediate Care Services in South East Essex. Community services are changing and developing rapidly with the introduction of Frailty virtual wards and Community Stroke Services. Our Intermediate Care Physiotherapists work as part of multidisciplinary teams alongside occupational therapists, speech and language therapists, rehabilitation assistants, nurses, psychologists and associate practitioners. Therapists have the opportunity to make a real difference to patients’ functional abilities in their own home or community settings, with therapy focused on individual goals rather than traditional discharge constraints.
There are opportunities to develop and specialise clinical skills in orthopaedics, neuro/stroke rehabilitation, falls prevention and frailty patients, as well as to develop leadership and managerial skills supporting new service development.
Responsibilities
The post holder is part of the Intermediate Care Physiotherapy team and will:
* Participate in the assessment, treatment and evaluation of patients in the defined area.
* Rotate between different clinical areas to enhance and specialise skills.
* Play an active role as a member of the Community Services team, participating in the planning, development and evaluation of Physiotherapy interventions.
* Be proactive in addressing their own continuing development needs.

* Inpatient Stroke Rehabilitation
* Early Supported Stroke Rehabilitation
* Frailty Virtual Ward / Admission Avoidance
* Re‑ablement and Unstable Fracture Management
* Collaborative Care Team – Unstable Fractures and Complex Neurological Rehabilitation
* Cumberlege Intermediate Care Centre – Inpatient Stroke Rehabilitation
Requirements
* Recently qualified physiotherapist with appropriate registration.
* Valid driving licence and access to a vehicle.
* Willingness to work within defined clinical areas and rotate as required.
