A forward-thinking, quality-focused independent case management company, specialising in catastrophic and complex injuries, is looking for a Clinical Case Manager with previous experience in the role.
What you’ll do as Clinical Case Manager?
You will provide support to either children and young people or adults with a range of needs, including traumatic and acquired brain injury, spinal cord injury, and amputation. You should be highly competent and dynamic in developing client-centred rehabilitation packages.
Some elements of the role can be completed remotely; however, this position also requires regular face-to-face meetings with clients and their families, support workers, and other members of the multi-disciplinary teams, as well as occasional attendance at the main office in Ipswich.
Staff development is supported with in-house training days and mentoring.
Main Responsibilities
* Coordinate services and resources for clients, ensuring timely access to necessary support.
* Maintain accurate and up-to-date case records and documentation.
* Monitor client progress and adjust rehabilitation plans as needed based on ongoing assessments.
* Advocate for clients by connecting them with appropriate resources and services.
* Collaborate with multidisciplinary teams to ensure comprehensive care delivery.
* Conduct regular follow-ups with clients to evaluate satisfaction and effectiveness of services provided.
* Provide emotional support and guidance to clients and their families throughout the case management process.
Skills and Experience
* A background in Occupational Therapy, Nursing, Physiotherapy, Speech and Language Therapy, Social Work, or Clinical Psychology.
* Registration with HCPC, NMC, Social Work England, or the IRCM.
* At least 2 years of experience as a Clinical Case Manager.
* Strong organisational skills with the ability to manage multiple cases effectively.
* Excellent communication skills, both verbal and written, for clear interaction with clients and professionals and effective report writing.
* Empathy and understanding towards clients' needs and challenges.
* Problem-solving skills to navigate complex situations and find suitable solutions.
* Proficiency in IT for documentation and reporting purposes.
