Experienced Social Worker\n\nLocation: Civic Campus, Euclid Street, SN1 2JG\n\nStart Date: ASAP\n\nContract Duration: 3+ Months\n\nWorking Hours: Mon – Fri, 09:00 – 17:00, 37 Hours per week\n\nPay Rate: £ 30.15 per hour \n\nJob Ref: OR9837\n\nThis role is situated within our Leaving Care Service, where the primary responsibility is managing a caseload of young people preparing to leave care. The position involves collaboration with allocated Pathway Advisors who are also part of our service team. Early engagement with young people transitioning from other services is a key aspect, and you will take the lead on completing pathway plan assessments and the initial pathway plan.
Maintaining these plans in compliance with the required legislation until the young person reaches 18 years old is a crucial responsibility, at which point they fully transition to their allocated Pathway Advisor. Additionally, you will participate in our duty system, providing extra support to our young people alongside our Pathway Advisors.\n\nResponsibilities\n\nManage a caseload of young people preparing to leave care.\n\t\nCollaborate with Pathway Advisors to support young people.\n\t\nLead the completion of pathway plan assessments and initial pathway plans.\n\t\nMaintain pathway plans in accordance with legislative requirements until the young person turns 18.\n\t\nParticipate in the duty system to provide additional support.\n\nPerson Specification\n\nProven experience in social work, particularly with young people transitioning out of care.\n\t\nStrong understanding of legislation related to care leavers.\n\t\nExcellent communication and collaboration skills.\n\t\nAbility to manage a diverse caseload effectively.\n\t\nCommitment to supporting young people in achieving positive futures.\n\nDISCLAIMER: By applying for this vacancy, you consent to your personal information being shared with our client and any relevant third parties we engage with, for the purpose of assessing your suitability for the role.
