Are you looking to be a part of an exciting new model of care? If so, we have a fantastic opportunity for an experienced Nurse Advanced Clinical Practitioner (ACP) to bring their expertise and knowledge to assist in the development, implementation, and ongoing delivery of clinical care as part of the Integrated Virtual Ward Clinical Team.
Responsibilities
* Work closely with the Acute Frailty Virtual Ward multidisciplinary team (MDT) to ensure all patients receive the best possible care and service.
* Provide autonomous assessment and treatment across the BHR area, including acute frailty and emergency departments, care homes and community.
* Assist in the development, implementation, and ongoing delivery of clinical care for the Integrated Virtual Ward Clinical Team.
* Participate in the virtual ward admission process and expand patient admission criteria.
Qualifications
* Experienced ACP with advanced clinical assessment and treatment skills.
* Knowledge of remote patient monitoring and digital technology in patient care.
* Experience working autonomously across acute frailty, emergency departments, and community settings.
