Overview

We have an opportunity for a full-time Social Worker in our Young Adults Team, for 37 hours a week. The Young Adults Team supports Adults aged 18-25 with a diagnosed learning disability who have an EHC Plan. The team works countywide from Scott House in Huntingdon, with remote working options and an office in Huntingdon. The team collaborates closely to promote personalisation, independence, choice, and opportunities for young people to develop their independence for adulthood. Occasional out-of-county travel may be required.

Part of the role involves working alongside Children’s services with young people under 18 to prepare them and their families for adulthood. You will support young adults through change, encourage progression, and acknowledge individual, family, and community strengths. You will work alongside Adult Learning Disability services, particularly health colleagues, and closely with Education services.


Responsibilities

* Complete Social Care Assessments, Social Care Reviews, and Care and Support Planning in line with the Care Act 2014.
* Undertake Section 42 enquiries with oversight from a Senior or Team Manager.
* Undertake Mental Capacity Assessments and be involved in Best Interest Decisions, including completing Concerning People DoL (CoP DoL) work.
* Collaborate with Children’s services for young people under 18 and with Adult LD services, health colleagues, and Education services.
* Promote the principles of personalisation, encouraging independence, choice, and opportunities for young people to develop independence for adulthood.
* Work as part of a dynamic, supportive team and contribute to a positive, inclusive working environment.


Qualifications

* Be a qualified Social Worker with a Social Work England registration.
* Hold a driving licence and have the ability to work from a variety of locations and travel countywide independently.
* Knowledge and experience of working with people with a learning disability is desirable but not essential.
* Working knowledge of the Care Act 2014 and Mental Capacity Act 2005 is important, as well as experience in safeguarding and managing complex casework in previous roles.
