Overview
To work as directed by a supervisor (e.g. Social Worker, Nurse, Occupational Therapist, Advanced Practitioner or Lead Professional) with people who may require care and support to identify needs, strengths and outcomes and provide a range of services to support people to manage their needs and maximise their independence. Managing a supervised caseload, you will work in a strengths-based way to achieve positive outcomes for people, ensuring people are as safe as possible and adult safeguarding concerns are addressed promptly.
Responsibilities
* Work within the policies and practices of the Adult Care and Community Wellbeing Directorate to prescribed standards and participate in learning and development as required to ensure Continuous Professional Development.
* Hold and manage a caseload of service users with a higher level of need, including identification of people and provision of information and advice. May include complex response assessment of the person(s) and carers need, commissioning follow-on care, monitoring and review, and amending as required. Operate within the budget framework set by the Directorate and managed by the Lead Practitioner.
* Provide support to Qualified Social Workers / therapists / nurses as required in line with the service roles and responsibilities.
* Other duties as specified on the job description.
