About the Role

As a Provider Support Assessor, you'll play a vital role in assessing people's care and support needs and developing reablement programmes for adults, including people leaving hospital and people with complex and changing needs. You'll work closely with individuals, carers, Home First - Community Service colleagues and professionals to promote independence, choice, and social inclusion.

What You'll Do

* Carry out assessments and develop tailored support plans.
* Monitor and review reablement programmes.
* Work collaboratively with internal and multi-agency colleagues.
* Maintain accurate records and contribute to service planning.
* Support service users to access community resources and build life skills.

What We're Looking For

* Experience in working with people, possibly in adult social care, particularly with older people or those with complex needs
* Strong communication and teamwork skills
* Knowledge of CQC standards
* A recognised Health or Social Care qualification (NVQ/QCF Level 3 or equivalent) or willingness to work towards one
* A commitment to equality, dignity in care, and continuous personal development
* Excellent computer skills
* Own car driver

Full-time employees work 37 hours for 52 weeks of the year and we offer a generous holiday entitlement. We are open to discussions about a wide range of flexible working opportunities which benefit you and the Council, including reduced hour contracts, working part time or as a job share.

If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
