Overview

Fixed term for 1 year. We are seeking an individual to work as part of the MS Team to support patients through pathways, monitoring progress and identifying potential breach patients — taking appropriate action to avoid these. You will also be involved in auditing the services provided. The post holder will work with a team of Clinical Nurse Specialists (CNS) to cover the MS Service. We are recruiting an individual to support patients going through different treatment and surveillance pathways. You will be based at Victoria Hospital, Kirkcaldy and will work collaboratively with the MS team to provide a seamless service, which is responsive to individual patient’s needs. You will have a key role in ensuring that patients have a confirmed diagnosis as early as possible, supporting those patients in all stages of the pathway by being a vital point of contact.

You will be part of a team to ensure that all administrative/assessment functions for patients from the time of GP/Acute Services referral to the time of discharge are coordinated in line with specific care pathways to facilitate the smooth running of the patients’ journey.


Responsibilities

* Support patients through MS treatment and surveillance pathways, monitor progress and identify potential breach patients, taking appropriate action to avoid breaches.
* Audit the services provided and contribute to service improvement.
* Collaborate with a team of Clinical Nurse Specialists (CNS) to cover the MS Service and support patients through different treatment and surveillance pathways.
* Be based at Victoria Hospital, Kirkcaldy and work with the MS team to provide a seamless, responsive service tailored to individual patient needs.
* Ensure that patients have a confirmed diagnosis as early as possible and act as a vital point of contact throughout the pathway.
* Coordinate administrative/assessment functions from referral to discharge in line with specific care pathways to facilitate the patient journey.
* Utilise excellent verbal and written communication to coordinate appointments and information required by clinicians in the pathway, and to support patients and their carers.


Qualifications and Skills

* Excellent verbal and written communication skills; empathic approach to working with patients and carers.
* Excellent IT skills with working knowledge of Excel and database management; knowledge of NHS IT systems is desirable.
* Excellent interpersonal and organisational skills; flexible, self-motivated, able to work under pressure and prioritise tasks/objectives.
* Ability to work as part of a friendly but very busy team/service.
* Secondment may be possible, but authorisation and support from the line manager must be sought before application submission.
* Fixed Term/Secondment Opportunity: existing permanent employees should discuss with their substantive line manager. Secondment policy details available within organisational communications.
