Hours: Part-time up to 24 hours per week
Salary: Starting Rate £13.50 per hour – Depending on experience

About the Role
We are looking for a dedicated and compassionate Social Prescribing Link Worker to join our team. This role supports individuals to take control of their health and wellbeing by focusing on the question, “What matters to me?”
Working closely with patients, you’ll help develop personalised care plans and connect individuals to community services, voluntary organisations, and other support networks. You will also collaborate with a variety of other agencies to strengthen the community resources available for social prescribing.
This role addresses broader determinants of health—such as social isolation, poor housing, financial concerns, physical inactivity, and mild mental health challenges. It is especially beneficial for individuals managing long-term conditions, facing social isolation, or dealing with complex social issues.
In line with your manager and the Senior Social Prescriber, you will manage your own schedule, dividing time between GP surgeries, home visits, community settings, and administration. While you’ll work autonomously with patients, you’ll be supported by your manager and the wider healthcare team.

You will also need to be able to work independently whist attending multidisciplinary team meetings. A sound understanding of local health, social care, and voluntary sector services is beneficial.
Key Responsibilities
* Build trusting, supportive relationships with patients
* Develop and implement personalised care and support plans
* Signpost and connect individuals to community-based resources
* Support the growth and sustainability of local voluntary and community groups
* Work collaboratively with GPs, healthcare professionals, and practice staff
* Attend multidisciplinary team (MDT) meetings and maintain patient records
* Contribute to the ongoing mapping and development of local support services
* Comprehensive induction and training
* 5 weeks annual leave (plus bank holidays)

Requirements
* Full UK Driving Licence and access to own transport
* Flexibility to work across multiple sites
