The skillset of a community nurse is vast and non-exhaustive requiring knowledge and expertise in a range of areas. Examples of interventions provided are wound management, continence care, palliative care, medication administration and long term condition management. Close working with the MDT (Social Workers, GP's and Allied Health Professionals) to ensure that patient care is effectively co-ordinated, demonstrates a positive impact on our patients.
