About the Role

Empower a caseload of service users to manage their health and wellbeing. As part of a team, you will support a staffing rota offering support to service users between the hours of 8am-10pm Mon-Sun each week. In collaboration with your team, you will actively monitor wellbeing, mental health, housing and living environment to ensure service users journey into independence. Moreover, you will take a solution focused and co-produced approach to supporting service users with their move-on into independent living.

Sounds great, what will I be doing?

This role involves empowering service users to build independence and engage confidently with professionals in the community, such as mental health teams, social workers, housing providers, and employment or training services. You will support continuous assessments of service users' needs, advocate for appropriate support levels, and help guide them through transitions in their journey toward independence. The role requires flexible teamwork to ensure service coverage and meet contractual obligations. You will co-produce group activities and peer support networks, encouraging service users to participate in community-based initiatives that align with their goals and aspirations. Supporting individuals to build local support networks, engage in meaningful activities, and develop independent living skills is central to the role.

You will be responsible for managing timely referrals, assessments, and service reviews, while also monitoring the health, wellbeing, and safety of users and responding promptly to concerns. Core responsibilities include contributing to daily service operations, supporting colleagues, completing administrative tasks efficiently and confidentially, and following organisational policies. You will also assist users in managing their health, tenancy, and financial responsibilities, such as paying rent and service charges. Additionally, you will support property management, including health and safety, repairs, and voids, while clearly communicating the aims and model of the service to promote active participation and co-production with service users.

Here's what the team will be looking for:

The role requires experience working with individuals facing mental health challenges and/or dual diagnosis, either in a paid or voluntary capacity. It demands the ability to quickly learn, absorb information, and make sound decisions, along with the capability to accurately receive, record, and share detailed information in line with confidentiality guidelines. You must be confident in working with individuals who may display challenging behaviours, using de-escalation techniques to manage potentially difficult situations. A solution-focused mindset and a collaborative approach to problem-solving are essential, as is an optimistic belief in people's ability to change. A calm, non-judgemental, and anti-discriminatory attitude is critical, especially when working in high-pressure or chaotic environments. The role also requires flexibility and a willingness to work unsociable hours, including early mornings, late evenings, and occasional weekends as part of a team rota. A solid understanding of safeguarding and the ability to respond to related concerns appropriately is also vital.
