We’re looking for an enthusiastic, passionate, and motivated Community Support Worker to join our friendly and forward-thinking Community Adults Mental Health Service in Sleaford.
The successful post holder will be an integral part of the service, responsible for providing safe, personalized care and support to people living in the community with severe and enduring mental health needs. Working alongside a multidisciplinary team, under the indirect supervision of registered clinicians, you will form compassionate, recovery-focused, therapeutic relationships with service users and their carers, supporting them to manage their mental health needs in a community setting. A key part of the role will be empowering service users to self-manage aspects of their ongoing treatment, such as medication, diet and lifestyle, physical health, and general wellbeing.

1. Observe and monitor mental state and levels of functioning, reporting observations to qualified mental health staff.
2. Provide social and emotional support to patients and carers within trusting and therapeutic relationships.
3. Encourage social contacts and promote social networks when appropriate, helping patients develop and maintain social skills.
4. Assist in assessing and meeting the care needs of patients and their carers in the community.
5. Participate in individual therapies in patients' homes and community settings, contributing to the care plan under supervision.
6. Liaise with other agencies in statutory and voluntary sectors.
7. Organize own daily work schedule to meet clinical needs.
8. Maintain clinical records according to trust policy.
9. Participate in supervision and appraisal processes.
