Job Summary
Are you an experienced Advanced Nurse Practitioner looking to make a real impact in primary care? We are seeking a dynamic, compassionate ANP to join our multidisciplinary team and help us deliver high-quality, patient-centred care.
Main duties of the job
* Deliver a high standard of patient care using advanced autonomous clinical skills and a broad theoretical knowledge base.
* Work in collaboration with GPs and other health professionals to provide comprehensive healthcare sensitive to the needs of the local population.
* Manage your own clinical workload in general practice, working within professional boundaries and responding effectively to patient need.

Job Responsibilities
* Provide advanced clinical assessment, diagnosis, and treatment for patients with acute and chronic conditions
* Prescribe medications and manage care plans independently
* Support urgent care and same-day appointments
* Home visits
* Support the GPs by carrying out ward rounds to local care homes
* Routine Practice Nurse duties
* Contribute to QOF, audits, and clinical governance
* Work closely with GPs and the wider team to improve patient outcomes
* Participate in safeguarding, risk management, and practice development
Person Specification
Qualifications
* Essential: Registered Nurse with active NMC registration
* Essential: MSc in Advanced Clinical Practice or equivalent qualification
* Essential: Independent Nurse Prescriber
* Essential: Full UK driving licence
Experience
* Essential: Experience of working in Primary Care
Desirable
* Experience with EMIS
Compliance
* Disclosure and Barring Service (DBS) check is required. This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975; a DBS check will be carried out.
