HCP 0-19 Public Health Practitioner

Are you highly motivated and keen to develop your skills working with children and families in the community? Join the 0-19 Healthy Child Programme as a fully qualified and experienced Registered Nurse or Midwife.
We require a current NMC registration and relevant experience. The role is based with the East of England Community Health and Care NHS Trust (Cambridge).
Responsibilities
* Deliver services as part of the 0-19 years Healthy Child Programme, working under the direction of the HCP Team Leader.
* Manage own caseload of children and young people across a range of settings (schools, clinics, client homes, community venues).
* Provide Universal New Birth contacts, 6‑week checks, 1‑ and 2‑year reviews, transfer‑in contacts, and school health assessments as competency allows.
* Support the delivery of public health group activities and themed drop‑in sessions.
* Attend events to promote the service and maintain links with local multi‑agency providers.
* Collaborate with key agencies such as Children’s Services and Education to address health inequalities.
* Adhere to safeguarding procedures, attend relevant training, and participate in child protection processes as directed by a SCPHN qualified practitioner.
* Act as a practice supervisor or assessor for student nurses following approved training.
* Assist the Duty Clinician with Text Us, Duty Desk, and digital services on a rotational basis.
* Lead child health clinics and support the delivery of identified health priorities.
* Communicate effectively with children, young people, families, carers, and professionals in complex situations.
* Maintain strict confidentiality, comply with the Data Protection Act and Trust information governance policies.
* Participate in audits, supervision, emergency preparedness and clinical governance activities.
Qualifications & Training
* Current NMC Registration.
* Evidence of continued professional development.
* Sound knowledge of child development.
* Knowledge of the role of a Healthy Child Programme practitioner in family‑centred public health.
* Experience of academic study at level 6 in the last three years.
* Experience of supporting and supervising students in a practice or assessor role.
Experience
* Multi‑agency and partnership working.
* Delivering health promotion or public health programmes.
* Safeguarding adults and/or children.
* Previous work as a member of a health visiting, school nursing or other community‑based team.
* Participation in health surveillance programmes.
* Managing or leading staff.
Knowledge & Skills
* Ability to manage and prioritise own workload.
* Proven ability to work autonomously and as part of a team.
* Understanding of issues relating to diversity.

East of England Community Health and Care NHS Trust (Cambridge) – Salary: £32,073 to £39,043 per annum, pro‑rata.
