Bay Primary Care Network (PCN) continues to grow and we have an opportunity for a number of experienced Pharmacy Technicians to join us in developing this new role in Primary Care.

The role will see you integrating with our existing team of clinical and primary care pharmacists, pharmacy technician, medicines management team and the wider practice team.

Main duties of the job

The pharmacy technician will play animportant role, complementing clinical pharmacists, community pharmacists andother members of the PCN multi-disciplinary team, working under supervision ofthe PCN Pharmacist to ensure effective and efficient use of medicines. Pharmacytechnicians core role responsibilities will cover clinical, and technical andadministrative categories.

The purpose of the role is to leadimprovements to maximise safe, cost effective best practice in prescribing toimprove the quality of patient care. The post holder will help patients to getthe best from their medicines by switching medications to agreed and approvedprotocols, improving repeat prescribing processes, including promotion ofrepeat dispensing and online ordering, minimising clinical risk and aiming toreduce wasted medicines.

In addition, the post holder will beresponsible for encouraging the development of better understanding of theprinciples of medicines optimisation throughout the practice teams andpromoting good practice in line with therapeutic developments. This willinvolve assisting the PCN in achieving national requirements, NICEimplementation and utilisation of medicines optimisation initiatives.

Job responsibilities

Job Summary

The pharmacy technician role hasbecome vital to the medicines management team at Bay PCN and as a result wewould like to expand our team further. Working alongside clinical pharmacistsand other members of the PCN multi-disciplinary team the post-holder will be challengedto deliver outstanding patient-centred care.

The post-holder will also be supportedto further advance their skills and knowledge through completion of the Primary Care Pharmacy Educational Pathway (PCPEP).

The integral purpose of the role is tolead improvements across a range of priority areas and help deliver qualitypatient care through safe, cost-effective best practice. The core roleresponsibilities will cover clinical, technical and administrative categories.

In addition, the post-holder will beresponsible for encouraging the development of better understanding of theprinciples of medicines optimisation throughout the practice teams andpromoting good practice in line with therapeutic developments. This willinvolve assisting the PCN in achieving national requirements, NICEimplementation and utilisation of medicines optimisation initiatives.

It is a requirement of the role to be registered with the GPhC.

Job responsibilities

The Pharmacy technician will work under supervision and have core responsibilities in clinical, technical and administrative categories, including:

Clinical

Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients.

* Complete medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.
* As determined by the PCN, support medication reviews and medicines reconciliation for new care home patients and synchronising medicines for patient transfers between care settings, linking with local community pharmacies
* Support the Clinical Pharmacists in Structured Medication Reviews (SMRs) i.e. information gathering, risk identification and organising monitoring tests.
* Provide expertise where competent to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.
* Take a central role in shared care protocols and liaise with Clinical Pharmacists for more complex patients.
* Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.

Technical and Administrative

* Support the PCN multi-disciplinary team to ensure efficient medicines optimisation processes are being followed.
* Implement efficient ordering and return processes and reducing medication wastage.
* Provide training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS).
* Promotion of Electronic Repeat Dispensing (eRD) and online ordering.
* Develop relationships with other pharmacy professionals and members of the multi- disciplinary team to support integration across health and social care including primary care, community pharmacy, secondary care and mental health.
* Support practice reception teams in streaming general prescription requests, so as to allow GPs and clinical pharmacists to review the more clinically complex requests.
* Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings achieved by undertaking audits.
* Support the PCN to deliver on QIPP agenda, QOF and locally commissioned enhanced services
* Support the PCN in reviewing and developing practice policies for CQC requirements

Education, teaching and mentoring

* Completed, currently undertaking or be prepared to start an approved 15 month training pathway (Primary Care Pharmacy Educational Pathway (PCPEP)).
* Participate in regular performance review and take part in the annual appraisal review where personal learning requirements will be identified and in accordance with Practice requirements.
* Undertake further specialist training as required by service needs and the changing requirements of service provision.
* Provide supervision and coaching to others in the practice to help development of the practice team.
* Take part in the in-house education events, delivering sessions where appropriate

Delivering a quality service

* Maintain professional registration at all times.
* Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
* Be aware of data protection issues particularly within a GP surgery.
* Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information.
* Review and process data using accurate Read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes.
* Assist in the development and implementation of policies and procedures.
* Understand and apply legal issues that support the identification of vulnerable and abused children and adults and be aware of statutory child/vulnerable patients health procedures and local guidance.
* To attend and participate in Significant Event Audits.
* To work within the policies and procedures of the Practice and within statutory requirements.


Person Specification


Qualifications

* Registration with the General Pharmaceutical Council as a Pharmacy technician
* BTEC Pharmaceutical sciences, or NVQ level 3 (or equivalent experience)


Experience

* Evidence of commitment to continuing professional development
* Experience working as a qualified, registered pharmacy technician
* Experience of pharmaceutical technical procedures
* Working knowledge of standard operating procedures
* Knowledge of current legislation relating to controlled drugs
* Commitment to continuing professional development
* Computer literate in Word, Excel, email, and internet
* Experience of working in primary care
* Relevant theoretical and practical knowledge of primary care networks, General Practice and evidence-based medicine
* Conversant in medical terminology
* Ability to analyse/ interpret ePACT/ prescribing data
* Presentation skills and experience of using PowerPoint or equivalent
* Use of EMIS Web

£31,168 to £37,934 a yearSalary shown is pro rata based on 37.5 hours per week.
