Overview
Develop strategies to sustain wellbeing and help autistic adults live more independently. You will be responsible for carrying out statutory functions such as Care Act assessments, mental capacity assessments and safeguarding enquiries when required. You will work with families and carers, enabling them to access support and resources and completing carer\'s assessments when appropriate. This role is based with the Autism Intensive Support team at Hellesdon Hospital and covers the whole county.
Responsibilities
* Carry out statutory functions including Care Act assessments, mental capacity assessments and safeguarding enquiries when required.
* Work with autistic adults, their families and carers to access support and resources and complete carer\'s assessments where appropriate.
* Tailor approaches to the needs of autistic individuals and support those around them to interact effectively with autistic people.
* Represent Adult Social Services in a multi-disciplinary setting and bring a social work perspective to multi-agency working.
* Be a member of the Norwich mental health social work team and participate in regular supervision and peer support.
* Work directly with autistic adults and their families in community settings, alongside health colleagues and staff from other agencies.
* Develop excellent working relationships with colleagues in the Autism Intensive Support team to enable a multi-disciplinary approach.
* Maintain a focused caseload with relationship-based practice, incorporating intensive support where needed.
* Support autistic adults to access community resources, improve relationships, and prevent crisis situations.
* Share knowledge with other agencies and support practitioners in working with autistic adults.
* Act as a key member of the Adult Social Services autism champions network.
* Engage in regular in-person work in both office and community settings to build meaningful relationships with the people we support.
Qualifications and Experience
* At least two years\' post-qualification experience.
* Experience and knowledge of autism to tailor approaches to individual needs and support those around them to interact more effectively with autistic people.
* Confidence to represent Adult Social Services in a multi-disciplinary setting and contribute a social work perspective to multi-agency working.
Location and Structure
You will be based with the Autism Intensive Support team at Hellesdon Hospital. This small pilot team offers intensive psychology-led support to a small number of autistic adults who are frequent users of emergency and crisis services. Your role will require regular travel across the county and substantial time spent in the community, working with autistic people and their families, alongside health colleagues and other agencies. The role is offered on a fixed-term/secondment basis as part of an eighteen-month pilot to test and learn what works best in preventing longer-term care and support needs.
Team and Collaboration
You will develop excellent working relationships with colleagues in the Autism Intensive Support team to enable a multi-disciplinary approach. You will work with a focused caseload and reflect the need for intensive engagement with the people you support. You will also be a key member of the Adult Social Services autism champions network.
