We are looking for a motivated andexperiencedPractice Nurseto join our multidisciplinary teamand help us provide safe, high-quality care to our patients.
You will be working with our clinical teams to deliver thebest primary care healthcare service to our patients. More specifically, ourPractice Nurses are responsible for the normal primary care clinical nursingduties and nursing management / administrative work.
Main duties of the job
 * Managing long-term conditions (e.g. diabetes, asthma, COPD)through routine reviews and proactive care, conducting chronic disease clinicsdelivering patient care and making referrals to specialist / secondary carewhere required
 * Providing cervical cytology with subsequent resultprocessing
 * Routine nursing care according to clinical based evidence,NICE and NSF guidelines
 * Administer childhood immunisations, travel vaccines and flujabs
 * Requesting necessary pathology services
 * Provide contraceptive advice and support
 * Contributing to QOF and local incentive schemes
 * Promoting health education, self-management and preventativecare
 * Working closely with GPs, HCAs, Nursing Associates, andPharmacists to deliver joined-up care
 * Ensuring high standards of infection control and clinicaldocumentation
 * Support Quality and Outcomes Framework (QOF) delivery andother enhanced services.
The right candidate will demonstrate strong interpersonaland communication skills for dealing with our diverse patient population and beable to work well in a close-knit team of both clinicians and non-clinicians.Strong clinical knowledge and good analytical skills are important to ensuresuccessful consultations.Minimum requirements: You must have a current NMC registration, and we wouldideally be looking for someone with experience working in a GP Primary Caresetting. Previous experience working with EMIS also preferred but not required.
