Offer description

The post holder will be involved with the safe and secure handling of medicines assessments (medicines security), analysing prescribing data into the Practice clinical team.

The post holder will work closely with the Practice doctors to support the delivery of a high standard of medicines management at Railway Medical Group. The post holder will also liaise with a wide range of staff within the practice, patients, local care homes, pharmacies, and local hospital trusts.


Main duties of the job

1. Reconciliation of medication on hospital discharge letters and clinic letters.
2. Updating patient records and liaising with patients and pharmacies where necessary.
3. Communicating information about medicines, such as their common drug dosage and use, to patients, care workers, nurses, and medical doctors.
4. Communicating medicines management and other technical information, or addressing identified issues or gaps, to a wide range of people including nurses, allied healthcare professionals, and doctors.
5. Reducing waste by decreasing inappropriate issue of prescriptions.
6. Ensuring review and authorisation dates are actioned and updated daily.
7. Promoting and coordinating the Repeat Dispensing Scheme within the practice as appropriate.
8. Training practice receptionists dealing with repeat prescribing to make the best use of their skills.
9. Assisting the prescriptions team regularly, depending on business needs.
10. Collecting data to monitor improvements aligned with core medicine management objectives and undertaking clinical audits.
11. Encouraging joint working and facilitating staff integration towards common prescribing goals.
12. Engaging with local community pharmacists to improve communication and relations.
13. Attending relevant courses and training events.
14. Keeping abreast of all medicine management issues and updating practice protocols accordingly.
