Job overview

Oxford Health Community Hospitals are delighted to offer the opportunity for a part-time Band 6 Occupational Therapist (OT) within our service. If you are passionate about working in an adult inpatient physical rehabilitation setting, and award winning team, where our goal is to enhance our patient's quality of life and health and facilitate their safe discharge from hospital, then we are the team for you.

We would love to hear from any experienced Occupational Therapists who have the compassion, skills and knowledge to help us assess our patient's and deliver person centred interventions which impact daily activities and functioning and improve health and well being and return to independence.

We're a supportive, well-established, and knowledgeable community hospital multi-disciplinary team. You will receive quality individual, safeguarding and reflective practice supervision.

We are a progressive service keen to look forward and find ways to develop and improve the service. This role is based at our Didcot Community Hospital Site but there is also potential to be work in Wallingford Hospital Team to aid cross site cover.

This role includes visiting patient's houses across Oxfordshire and carrying out assessments and may also include delivery of small pieces of equipment to facilitate discharge.

Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.


Main duties of the job

This role would suit someone who is experienced in conducting specific physical and cognitive/functional assessments and delivering interventions to support rehabilitation. The post holder should be able to communicate this effectively within the MDT and family but also externally to key organisations such as social care and specialist services. You will also have responsibility for prescribing equipment and assessing home environments as appropriate. You will also be expected to represent and lead the service at family meetings, and be aware of the protocols and procedures in place to ensure clinical governance and adherence to standards.

It would be expected that the post holder has knowledge and experience of working with patients in a variety of specialties such as neurology, orthopaedics, dementia care, amputees, spinal injuries, and frailty.

Detailed job description and main responsibilities

Clinical Care Responsibilities may include:

* Delivery of a safe and effective rehabilitative patient care and discharge, working in partnership with colleagues in the multidisciplinary team within the community and acute sectors, supporting the efficient use of resources across the whole system delegating duties as appropriate.
* Participate in the hands on delivery of patient care.
* Work flexibly across localities and occasional weekends to ensure the requirements of the service contract are met.
* Be visible and accessible to patients, relatives and colleagues in order to work effectively within the team.
* Work independently as part of an interdisciplinary team to prioritise, assess, plan and implement patient specific OT programmes of care/rehabilitation,analysing patient needs and making judgements about best practice contributing to the continuous assessment of patients.
* Undertake assessment of patients as an autonomous practitioner using clinical reasoning skills, knowledge of evidence based practice and professional expertise. Determine appropriate care plans utilising expert treatment skills and options.
* Undertake practice education and support student placements


Person specification


Knowledge

* Previous NHS or Adult Social Care experience.
* Previous experience as Band 5 or equivalent.
* Experience of working within a multidisciplinary team.
* Good working knowledge of rehabilitation.
* Good knowledge of community services.


Qualifications

* Professional Qualification (degree or equivalent).
* Professional Registration with HCPC.
* Evidence of continuing professional development.
* Post graduate education/training specifically relating to patient group.

* Practice educators course or equivalent.
* Experience of supervising junior staff and /or students.


Experience

* Good knowledge and skills of elderly care/rehabilitation and proven specific clinical expertise.
* Understanding and experience of risk management.
* Good understanding of clinical governance.
* Excellent english verbal and written communication skills.
* Ability to function effectively across a variety of working environments.
* Aware of national and local issues concerning this patient group.
* Experience of supervising student placements.
* Understanding of audit process.
