Working as part of a passionate team to provide outstanding support to an incredible young gentleman with Learning Disabilities and Autism in Risca.
Responsibilities:
* Implementing Positive Behavioural Support (PBS) and personal plans, ensuring effectiveness and continuous improvement with the Multidisciplinary Team (MDT) and PBS Lead.
* Collaborating closely with family members and professionals to create a strong support network.
* Encouraging community involvement, helping individuals engage in local activities.
* Building essential life skills, including cooking, household tasks, managing finances, and medication administration.
* Providing support in the community - from shopping and appointments to education and leisure activities.
* Assisting with personal care as outlined in individual care plans.
* Contributing to risk assessments and support plans, ensuring a safe and empowering environment.
Requirements:
* A full UK manual driving licence (to support with mobility vehicle transport).
* Experience in care with a knowledge of Positive Behavioural Support.
* A compassionate and proactive approach to care.
* A passion for enabling independence - Helping individuals live life on their own terms.
* Enthusiasm and a desire to make a real difference - Positivity and energy go a long way!
* Strong initiative & teamwork skills - Be proactive, work independently, and collaborate effectively.
