Virtual Ward Community Practitioner

We are seeking an enthusiastic Senior Community Practitioner (Nurses) to join the Nottinghamshire Virtual Ward. This full‑time Band 6 position delivers hospital-level care at home using remote monitoring, digital tools and multidisciplinary community support. The role supports frailty, respiratory and admission avoidance pathways and works across Nottinghamshire Healthcare, NUH, Sherwood Forest Hospitals, Doncaster& Bassetlaw Teaching Hospitals and CityCare.
Main duties of the job
Deliver timely, person‑centred assessments and provide high‑quality clinical care to patients on the Virtual Ward for up to 14 days.
Work autonomously and within the MDT (including Matrons, Nurses, AHPs and support staff) to prevent unnecessary hospital admissions and support safe early discharge.
Interpret remote monitoring data, undertake face‑to‑face clinical reviews, identify deterioration and initiate appropriate interventions or escalation.
Lead and coordinate safe, effective discharge planning and contribute to daily MDT discussions on treatment and risk.
Support junior staff through supervision, role‑modelling and clinical guidance.
Maintain accurate documentation on SystmOne and uphold professional standards, governance and patient safety.
Work flexibly across respiratory, frailty and admission avoidance pathways, following lone‑worker and dynamic risk assessment procedures.
Job responsibilities
Provide care to patients in a community setting who require high‑quality holistic evidence‑based care that is responsive, effective and efficient within the virtual ward model.
Work flexibly over 7 days, including weekends and bank holidays (shifts 8 a.m. – 8 p.m.).
You will work within the Virtual Ward Team and collaborate with system partners such as City Care, in a supportive multi‑disciplinary clinical team.
Qualifications
* Level 1 Registered General Nurse.
* Evidence of working at a senior level.
* Degree‑level qualification or equivalent knowledge and experience in a health‑related subject (e.g., Specialist Practice Degree or Leadership Qualification).
* Advanced clinical assessment evidence (practical evidence or courses).
* Demonstrates continued professional development to maintain professional registration.
* Holds a mentorship qualification and can provide mentorship to others.
* End of Life Care.
* Frailty / LTC modules.
* Advanced clinical assessment courses (e.g., Dreams/Top To Toe course).
Experience
* Experience of working autonomously.
* Community or primary care experience.
* Caseload management and experience of triaging patients with a range of clinical conditions.
* Multi‑disciplinary working.
* Experience of supervising and appraising staff.
* Project management.
* Conflict management.
* Experience of close working relationships with statutory agencies and other external organisations.

Other Requirements
A full UK driving licence and a vehicle for business use are required for this post; reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

£39,959 to £48,117 a year per annum (pro rata for part time).
