We welcome applications for a qualified SCPHN Health Visitor to join our 0‑19 team in Northumberland. The post holder will be based in Ashington within an established skill mixed team. The team comprises Health Visitors, Public Health Staff Nurses and Family Health practitioners to deliver the 0 to 5 component of the Healthy Child Programme.
Responsibilities
The Health Visiting role is to deliver the Healthy Child Programme as part of a one team approach, planning, delivering and evaluating an efficient and cost‑effective service for an identified population. You will be delivering the National and Local Public Health agendas in response to identified health and social need working with partner agencies in a strategic, creative and holistic way to ensure best outcomes for children, young people and families.
The HV is responsible for leading and delivering the Healthy Child Programme against the six early years high impact areas, ensuring services are provided within the tiered model approach – building community capacity, universal, universal plus and universal partnership plus. Excellent interpersonal and communication skills and the ability to deliver under pressure and work in a rapidly changing environment are essential.
