If so, an exciting opportunity has arisenfor a 9 month temporary contract for GP Assistant to join our PCN. The successful applicants will workalongside the PCN team to support healthcare professionals, patients andcarers.

You will be part of an innovative multidisciplinaryteam working across the Bridgedale PCN to support and assist the GP practiceswithin the PCN.

We are looking for dynamic and pro-active GPAssistant on a fixed term 9 month contract to assist in improving patient outcomes and support the primary careworkforce in general practice.

Riverside Medical Centre

The Ryan Medical Centre

New Longton Surgery

Lostock Hall Medical Centre

Main duties of the job

Our PCN is looking for a GP Assistant to join our growing primary care workforce. The successful candidates will work across the patch, within the PCN Practices, CommunityTeams and across the Multi-Disciplinary Team (MDT).

A GP Assistant will deal with patients and, if appropriate, their carer, before or after the patients consultation with a clinician or other healthcare professional.

The GP Assistants role requires them to be able to work with, and understand the roles of, a variety of different people working in the practice and across the PCN including doctors, nurses, healthcare assistants, social prescribing link workers, physiotherapists, paramedics, health and wellbeing coaches, and pharmacy technicians.

A GPA will support the Care home team and GP(s) smooth running of clinics by performing the more routine administration and clinical tasks on behalf of the GP freeing up their time to focus on the patient. The GPA will also carry out some clinical tasks and reviews in the Care Homes to support the Care Home Clinical team.


About us

GP practices have been working together in our area for years and formed BridgedalePrimary Care Network (PCN).Bridegale Primary Care Network is made up of GP registered patient lists for Roslea Surgery, Riverside Medical Centre, Lostock Hall Medical Centre, New Longton Surgery and the Ryan Medical Centre. We serve a community of around 36,000 people. We are small enough to provide the personal care valued by our people and large enough to have impact and economies of scale through better collaboration.

As a Primary Care Network we are working hard to deliver on the national NHS England network contract that commenced in July 2019 and continues to March 2024.

This includes the following areas of focus:

* anticipatory care
* cardiovascular disease prevention and diagnosis
* enhancing health in care homes
* early diagnosis of cancer
* tackling neighbourhood inequalities
* structured medication reviews
* social prescribing reviews

So as a primary care network ,with its member practices working ever closer together, it is important to establish shared core values which not only enables us to adapt, but to do so in the way that we all feel is right for our patients and staff. The outstanding contributions from Ryan, Roslea and Riverside surgeries in developing Bridgedales SHINE values is a key milestone and prepares the ground for the future.

Job responsibilities

CORE RESPONSIBILITIES

* Sorting all clinical correspondence and prioritizing for the GP in terms of actions.
* Signposting some post to others such as clinical pharmacist etc.
* Arranging appointments, referrals, tests and follow up appointments of patients.
* Preparing patients prior to going in to see the GP, taking a brief history and basic readings in readiness for the GP appointment.
* Dipping urine, taking blood pressure, ECGs & phlebotomy
* Explaining treatment procedures to patients
* Helping the GP liaise with outside agencies e.g., getting an on-call doctor on the phone to ask advice or arrange admission while the GP can continue with their consultation(s)
* Support with QOF and IIF reviews.
* Support with New Patient Medicals, reviews and Health Checks in care homes

GENERAL

The post holder will:

* Ensure the principles of openness, transparency and candour are observed and upheld in all working practices.
* Have, or acquire through training provided by the organisation, the appropriate level of safeguarding and knowledge, skills and practice required for the post and be aware of and comply with the organisations safeguarding protection policies, procedures and information governance.
* Ensure that any infection prevention and control issues are reported to the line manager/Infection Prevention and Control Lead
* Ensure that all activities are monitored and evaluated.
* Attend internal meetings as required.
* Work in accordance with PCN policies, including the equal opportunities policy and practice.
* Attend any training courses and supervision sessions as required.
* Work with due regard to the Health and Safety Policy, ensuring that all practices and procedures are undertaken in accordance with the issued guidelines.
* To keep the MDT and the managers updated with both positive feedback and any verbal or written complaints.
* Is able to recognize personal limitations and refer to more appropriate colleagues when necessary
* The post holder will be expected to deliver a high quality, diverse, patient centered service.
* Undertake any other duties as may reasonably be required from time to time
* SPECIAL WORKING CONDITIONS
* To be prepared to work flexibly, including evenings, bank holidays and weekends on a rota basis.

* Minimum of 1 year experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field
* Experience of working in a multi-disciplinary setting where influence and negotiation is required
