Based at the Warracknabeal campus, conveniently located halfway between Melbourne and Adelaide, this part-time Intake Clinician position offers the opportunity to support community wellbeing in a supportive and progressive environment.
Role summary
As the Intake Clinician, you will work closely with the Intake Team to support consumers accessing a wide range of community health services. This includes initiating intake processes, creating episodes of care, scheduling appointments, and maintaining accurate client data and documentation.
Responsibilities
* Initiate intake processes and create episodes of care for consumers accessing community health services.
* Schedule appointments and maintain accurate client data and documentation.
* Collaborate with the Intake Team and across disciplines to support a person-centred approach.
Qualifications and experience
* Relevant qualifications as a Nurse, Allied Health Assistant, or Allied Health Professional
* Experience in person-directed care within a community setting
* Strong interpersonal and communication skills, both written and verbal
* Excellent organisation and time management skills
* A team-oriented approach with the ability to collaborate across disciplines
* Proficiency in digital systems and general computer skills
* Commitment to ongoing professional development
Desirable
* Knowledge of Primary Care and Chronic Disease Management principles
* Familiarity with Uniti (client management system)
* Understanding of accreditation programs and related legislation
Requirements
* Current Police Check, Working with Children Check and NDIS Worker Screening Check, or willingness to obtain
* Valid driver’s licence
* Immunisation history
