* Anticipate, coordinate, and integrate health and social care needs for patients at high risk of unplanned hospital admissions.
* Take a central role in holistic care planning collaborating closely with Social Care teams, voluntary sector partners, and other organisations.
* Assess patients who are medically optimised for discharge, identify their ongoing care needs, and make timely referrals to the appropriate community services.
* Play a key role in ensuring safe, effective, and timely discharges, supporting optimal patient flow throughout the hospital.
* Act as the primary liaison between the hospital and relevant community services—both formal and informal—to facilitate smooth and safe transfers between hospital and home.
