The Service
The Hospital in the Home (HITH) unit is an acute outreach service that delivers hospital treatment to patients in their place of residence. The HITH unit utilises one workforce and is also responsible for the delivery of care to patients receiving Post Acute Care and Chemotherapy in the home under our Cabrini at Home program.
The Role
We are currently seeking a Senior HITH Nurse to join our team on a 0.8 FTE basis, with consideration of alternative FTE options for the right candidate/s.
This position follows a rotating roster and includes some on-call responsibilities to ensure continuous, high-quality care for our patients in the home setting.
In this senior clinical role, you will work both independently and in collaboration with physicians, specialists, and healthcare professionals to provide exceptional, patient-focused care.
Key Responsibilities:
Develop and implement comprehensive care plans tailored to each patient’s needs
Monitor patient progress and ensure continuity of care
Conduct clinical assessments, administer treatments, and manage patient care
Educate patients and their families on health management and recovery strategies
Work in alignment with Cabrini Health’s policies, procedures, and core values

About you
To be successful in this role, you will possess:
Strong clinical expertise in delivering patient-centred care
Ability to work autonomously and manage patient care effectively
Proficient in handling clinical equipment and therapies, including wound care and IV and CVC management
Skilled in recognising and responding to clinical deterioration
Strong documentation and adherence to evidence based practices
Commitment to quality improvement and continuous professional development
Along with your passion for delivering a high standard of nursing care and an interest in further developing your skills, you will also have:
Current registration as a Registered Nurse with AHPRA.
Valid driver’s licence
Current Working with Children Check
Proven experience working within the acute and community nursing
