This is an exciting opportunity to work in the Navigo Rehabilitation Team, dedicated to supporting individuals in mental health rehabilitation, promoting empowerment and independence.
We use a variety of approaches to enable service users to improve their independence, safety, and quality of life. This includes working with individuals residing at Hope Court (community rehabilitation unit) and Brocklesby Lodge (inpatient rehabilitation), as well as in their own homes, providing practical and educational support and compensatory strategies to help them do what they need, want, and like to do. We create plans with people to support them in achieving their goals and aspirations.

1. Hold a caseload, managing it autonomously and using prioritisation skills to ensure people are seen in a timely manner to appropriately manage any risks.
2. Assess, develop, implement, and evaluate OT care plans using a person-centred approach with individuals using the service.
3. Liaise and communicate effectively with all parties involved in an individual's care, including consultants, AC/RC, social workers, nurses, Assistant Practitioners, carers, and family members.
4. Promote and develop innovative and creative ways of working using available resources to achieve the best possible care and outcomes.
